god, please learn how to spell. seriously, it only takes an extra 4 seconds to look up “expensive” on your word processing document. knowing you, you were probably wikipedia-ing new kids on the block or something.
any ways, the answer to your question can be found here. UTSU (i miss the old acronym by the way) has a detailed list of information of how to claim pills and other fun stuff as a student. the information is as follows:
UTSU (formerly SAC) MEMBERS
Your identification number at Green Shield is: ‘SAC’ + your UofT student number – ‘00’.
For example, a plan member’s identification number would appear as: SAC111222333-00. The ‘00’ indicates the primary plan member. ‘01’ is for a spouse, ‘02’, ‘03’ etc. for dependent children. (See “Family Coverage”.)
Because it is a reimbursement plan, there is no membership card. We receive regular lists of students from ROSI and these are used to verify eligibility.
Mail original receipts along with a completed SAC/UTSU claim form to Green Shield (keep a photocopy for your records). Their address is on the claim form. You will receive a cheque within 3 weeks. Please delay submitting your September and October receipts until November 1, when the opt-out and opt-in period ends and Green Shield receives the list of eligible students.
Claims Must:
• Include your student identification number (SAC + your student number – 00)
• Include original receipts
• Must be submitted within 12 months of service being rendered, or 90 days following the termination of your plan, whichever comes first.
We do not provide Green Shield with your date of birth. Please use 1/1/60, the generic date they have given our students, when asked by Green Shield for your date of birth.
The completed forms, receipts and documentation should be sent to the insurance company at:
Green Shield Canada
P.O. Box 1699
Windsor, Ontario N9A 7G6
IT IS YOUR RESPONSIBILITY TO KEEP COPIES OF EVERYTHING SUBMITTED.
If you need help making a claim, or have questions about your benefits, contact Green Shield @ 1-888-711- 1119.
ta da! and if you’d like to share some of your codeine or tylenol 3’s with a friend, please contact the innis college registrar. askastudent is NOT having a good valentine’s day weekend.
cheers, askastudent!
