\nUTSU (formerly SAC) MEMBERS
\nYour identification number at Green Shield is: \u2018SAC\u2019 + your UofT student number – \u201800\u2019.
\nFor example, a plan member\u2019s identification number would appear as: SAC111222333-00. The \u201800\u2019 indicates the primary plan member. \u201801\u2019 is for a spouse, \u201802\u2019, \u201803\u2019 etc. for dependent children. (See “Family Coverage”.)
\nBecause it is a reimbursement plan, there is no membership card. We receive regular lists of students from ROSI and these are used to verify eligibility.
\nMail original receipts along with a completed SAC\/UTSU claim form to Green Shield (keep a photocopy for your records). Their address is on the claim form. You will receive a cheque within 3 weeks. Please delay submitting your September and October receipts until November 1, when the opt-out and opt-in period ends and Green Shield receives the list of eligible students.
\nClaims Must:
\n\u2022 Include your student identification number (SAC + your student number – 00)
\n\u2022 Include original receipts
\n\u2022 Must be submitted within 12 months of service being rendered, or 90 days following the termination of your plan, whichever comes first.
\nWe do not provide Green Shield with your date of birth. Please use 1\/1\/60, the generic date they have given our students, when asked by Green Shield for your date of birth.
